Iron Studies Pattern Interpreter
Classify iron studies into IDA, anemia of chronic disease, mixed, iron overload, or thalassemia/normal.
Iron
mcg/dL
Normal 60–170 mcg/dL
TIBC
mcg/dL
Normal 240–450 mcg/dL
Ferritin
ng/mL
Normal 30–400 ng/mL
Iron deficiency anemia (IDA)
Low ferritin (<30) and low transferrin saturation (<20%) with high TIBC are diagnostic of iron deficiency. No further iron studies needed; investigate cause (GI loss, menstrual loss, malabsorption, dietary).
Tsat
7.3%
Considerations
- Look for an underlying source of GI blood loss (colonoscopy/EGD) in adults without an obvious cause.
- Oral iron is first-line; IV iron when malabsorption, intolerance, or rapid replacement needed.
| Pattern | Iron | TIBC | Ferritin | Tsat |
|---|---|---|---|---|
| IDA | ↓ | ↑ | ↓ | ↓ |
| ACD / inflammation | ↓ | ↓ / N | ↑ | N / ↓ |
| Iron overload | ↑ | ↓ | ↑↑ | ↑↑ |
| Thalassemia trait | N | N | N | N |
