Last dose timing

Take the last DOAC dose 24 hours before the procedure (or earlier). Plan accordingly: a once-daily DOAC at a 24-hour hold means skipping 1 dose; at 48 hours, skip 2.

No bridging needed for DOACs

Routine bridging anticoagulation (LMWH) is not recommended for patients on DOACs around elective procedures. Their pharmacokinetics are predictable and shorter-acting than warfarin. Bridging adds bleeding risk without clear benefit in this population.
