|---|---|---|
| Furosemide | 40mg | 80mg |
| Torsemide | 40mg | 40mg |
| Bumetanide | 2mg | 2mg |
Bioavailability matters here
Furosemide IV:PO = 1:2 (~50% oral bioavailability, worse with gut edema). Torsemide and bumetanide are 1:1.
Quick reference
- Furosemide 40 mg PO = 20 mg IV = Torsemide 20 mg = Bumetanide 1 mg
- Furosemide bioavailability: ~50% (highly variable, worse with gut edema)
- Torsemide bioavailability: >80%
- Bumetanide bioavailability: ~80%
TRANSFORM-HF (JAMA 2023) showed no significant difference in all-cause mortality between torsemide and furosemide in HF. The DOSE trial (NEJM 2011) found no difference between continuous infusion and intermittent bolus dosing.
